Apr 16th is Advance Care Planning Day in Canada. Join us as we welcome Alberta Health Services to discuss how to communicate and document your wishes. Information on accessing home care services, assisted living, and planning for continuing care will also be provided. Family members are welcome. Light lunch provided.
